Shenzhen residents can now apply for health declaration cards and caregiver permits online without filling out paper-based epidemiological history declaration forms. The digital forms can be available within 24 hours, DT News reported.
To apply for health declaration cards, patients, guardians and caregivers need to follow their respective hospital’s official WeChat accounts, choose “health declaration cards” service or scan the “digital health declaration cards” QR code and then fill in the required information on the forms. They must present these digital health declaration cards and codes to medical staff before entering the medical facilities.
For pediatric patients, parents accompanying them have to fill out the forms by providing the child’s and care providers’ information, aside from their personal information. Their health declaration cards will be valid for both the child and the parents.
For patients who cannot apply by themselves, relatives and medical staff can complete the application on their behalf.
Relatives must scan a QR code at hospitals for digital caregivers’ permit when they apply for authorization to accompany patients. They have to fill out personal declaration forms, submit nucleic acid tests taken within seven days and submit their ID photos. The digital permits will be issued after they have been examined by nurses. Patients’ relatives will need to present these permits to medical staff before entering medical facilities and they can reapply once these permits expire.
